All coaches, managers and assitant managers are required to complete a Risk Management Form "A" before they are eligable to coach or interact with players.

Any persons dealing with money for their teams must complete the Risk Management Form "B".

Once forms are completed, they are to be send to the soccer office in a SEALED envelope to the ATTENTION RISK MANAGEMENT.

Any persons not compliant with completion of Risk Management will be removed from the team, until form is completed.
