All Referees for U11 to U18 games please click on the link below to submit game reports, caution reports and sending off reports. 

It will direct you to the NYDSA website where there is a selection of game reports for the different clubs and a caution and sending off report.

As part of your referee duties you are required to submit a game report for each game you ref in the U11-18 league.  This will ensure you are paid for your time.
